Health Plan of San Joaquin Marks 30 Years of Helping Local Families Access Care

For three decades, Health Plan of San Joaquin has been part of the local healthcare safety net, helping families, seniors, children, and individuals across the region connect with the care and services they need.

This year, Health Plan of San Joaquin/Mountain Valley Health Plan is celebrating its 30th anniversary, marking a major milestone for an organization that began in 1996 as a locally established health plan serving about 40,000 members in San Joaquin County. Today, the Health Plan serves more than 385,000 members across Alpine, El Dorado, San Joaquin, and Stanislaus counties.

That growth tells a larger story about the changing healthcare needs of local communities and the importance of keeping care accessible, local, and connected to real-life needs.

Over the years, Health Plan has expanded with the communities it serves. In 2014, it began serving Stanislaus County, opening the door for thousands more residents to access care. In 2015, the organization earned accreditation from the National Committee for Quality Assurance, reflecting its commitment to quality and accountability.

More recently, Health Plan expanded into Alpine and El Dorado counties as Mountain Valley Health Plan and launched a Dual Eligible Special Needs Plan, known as D-SNP, to better coordinate care for people who qualify for both Medi-Cal and Medicare.

Lizeth Granados, Chief Executive Officer of Health Plan, called the 30-year anniversary “a profound milestone” and said the organization remains committed to being a trusted local partner for individuals and families.

Beyond doctor visits and health coverage, Health Plan’s work also reaches into the everyday factors that affect health, including food access, housing stability, preventive care, chronic disease management, and culturally responsive services. Through partnerships with community-based organizations, healthcare providers, and local agencies, the Health Plan continues working to remove barriers that can keep people from getting timely care.

Dr. Lakshmi Dhanvanthari, Chief Medical Officer for Health Plan, said the organization’s dedication to community health continues to guide its investments in local programs, provider support, and long-term health solutions.

As Health Plan looks back on 30 years of service, its focus is also on the future. Throughout the year, the organization plans to share stories of community impact, recognize local partnerships, and provide resources to help members stay informed and connected to care.

For many families in San Joaquin and surrounding counties, access to healthcare is more than a benefit. It is peace of mind. After 30 years, Health Plan of San Joaquin/Mountain Valley Health Plan says its mission remains centered on helping local residents get the care and support they need to live healthier lives.
